You may pick up to 3 additional fields to be printed on your card from the options below.

image must be in .gif, .jpg or .png format, at least 600 x 480 px, with a maxium size of 2MBytes. It would be best if you re-submit the original photo you use in your Facebook profile, but you may use any photo you like. We do not keep a copy of it on our webserver. You must have copyright access to the photo you use!

In order to provide a measure of security, we can not override a Facebook name or a Facebook UserID.
